Telehealth Consent

1. Telehealth Services

You understand that Revel Health facilitates access to healthcare services delivered via telehealth, which may include online health questionnaires, secure messaging, phone calls, and/or video consultations with licensed healthcare providers.

Telehealth allows providers to evaluate patients remotely but may not be appropriate for all conditions. Telehealth does not replace in-person care when such care is medically necessary.

2. Independent Medical Providers

You acknowledge that medical services are provided by independent, licensed healthcare providers who are not employees or agents of Revel Health.

Revel Health operates as a management services organization (MSO) and provides non-clinical administrative, technology, and care coordination services only. Revel Health does not practice medicine, provide medical advice, or make treatment decisions.

3. Provider Evaluation and Treatment Decisions

You understand and agree that:

  • Completion of an intake form or submission of payment does not guarantee approval for treatment.

  • All medical decisions are made solely by a licensed healthcare provider based on medical appropriateness.

  • A provider may determine that you are not eligible for treatment or may recommend alternative care

4. Risks and Limitations of Telehealth

You understand that telehealth has limitations, including:

  • The absence of an in-person physical examination

  • Reliance on the information you provide

  • Potential technical or communication issues

You agree to seek immediate emergency care by calling 911 if you believe you are experiencing a medical emergency.

5. Privacy and Electronic Communications

You consent to receive communications electronically, including email, text messages, and secure portal communications, related to your account, services, and care coordination.

You acknowledge that electronic communications may carry inherent privacy risks and agree to review Revel Health’s Privacy Policy for additional information.

6. Right to Withdraw Consent

You may withdraw your consent to receive telehealth services at any time by discontinuing use of the platform or contacting Revel Health support. Withdrawal of consent may limit your ability to receive services through Revel Health.

7. Acknowledgment and Consent

By continuing, you confirm that you:

  • Have read and understand this Telehealth Consent

  • Voluntarily consent to receive telehealth services

  • Understand that medical services are provided by independent licensed providers

  • Agree to Revel Health’s Telehealth Consent, Terms of Service, and Privacy Policy
